Definitions of failure:
Tracey: Not doing anything about your situation
Wil: Not doing everything I can to help my clients be successful
Greg: Bad communication, false assumptions and expectations of work created and one another
Two kinds of failure....
1) recoverable. Needs course correction
2) unrecoverable. Abandon ship or drown
Greg: 80% of issues are scope-related
Tracey: Easy to place yourself in the shoes of the user without actually knowing them.
Tracey: Didn't test e-mail module in furniture site CMS. Didn't censor addresses in To: field
Tracey "When you get email from client with the word fuck in it, you know you fucked up"
Tracey: important to take the repremanding from client and not try to fix mode right away

Greg: Gone thru 6 revs of design, client not happy. Expensive. Take creative simple solutions, do low fidelity.
